All students entering or re-entering the B.S./M.S. degree in Athletic Training are required by New York State law to:
- Submit a Health Examinations/Vaccinations form to University Health & Medical Services. This form can be found in the Forms & Documents section of the Web site.
- Athletic training students in the Professional Phase of the ATEP are required to annually submit a health services receipt as proof of a completed physical exam no later than September 15th of each year to the clinical coordinator. Physicals are to be completed by the student’s health care provider or by University Health and Medical Services.
Students are required to submit a Clinical Affiliation Physical Examination form, which also is available from the clinical coordinator in addition to being printed in the appendix of the Athletic Training Handbook. Only verification of the physical and immunizations will be kept in the student’s file within the Division of Sports Sciences ATEP. At no time will medical records be stored in the academic office. Students should ALWAYS retain copies of completed physicals and immunizations as they may be required to observe surgeries or to visit or work at clinical sites.
