Thank you for using the Direct Payment Request form. We have received some feedback relating to pop-up messages that may be causing some confusion. We apologize for the confusion and thank you for your patience.
If the following message appears at the top of your screen: Click “Sign” to fill out this form. When you are done, you can save a copy by clicking “Done Signing."
Then click “Sign” in the top right corner.
The following pop-up message should appear. Click on the “Don’t show again” check box and click close. You will now be able to fill the form out, print the form, and circulate for approval signatures.
If the message box: “Cannot save form information” appears. Then click on the “Don’t show again” check box and click close. You will now be able to fill the form out, print the form and circulate for approval signatures.
